St. Bonaventure University strongly encourages the reporting of all bias incidents that occur on campus or at university-sponsored events or activities occurring off campus. A bias incident is characterized as the use of language and/or actions that demonstrate bias against an individual or group based on actual or perceived characteristics such as race, religious belief, sex, marital status, sexual orientation, gender identity or expression, national or ethnic origin, disability, veteran status, or age.
If you are witness to a crime or act of violence in progress, please contact Safety and Security Services immediately, 716-375-2525 (on-campus) or 911 (off-campus). This incident reporting form is meant to alert campus officials of incidents or issues we may not be aware of, or provide supporting documentation for incidents/crimes that have occurred. This report will go to the members of the Bias Incident Response Team (BIRT), as well as the Vice President for Student Affairs. If you are asking to be contacted or provide your name, someone from the University will contact you within 24 hours. This form is NOT an immediate alert, and all emergency situations should be reported directly to Safety and Security or 911.
